- Bruce Willis and director Len Wiseman deliberately minimized CGI usage throughout the film to maintain a grounded, practical feel rather than relying on the computer-heavy spectacle of contemporary action films.
- A stunt double for Maggie Q accidentally kicked Bruce Willis above his right eye while wearing stiletto heels during a fight scene, resulting in a wound so severe that director Len Wiseman could see bone beneath the skin.
- Bruce Willis received seven stitches through his right eyebrow and into the corner of his eye, and due to the film's non-linear shooting schedule, these stitches are accidentally visible in the scene where McClane first delivers Farrell to Bowman.
- Bruce Willis' stunt double Larry Rippenkroeger fell 25 feet from a fire escape and suffered a punctured lung, broken facial bones, multiple broken ribs, and fractured wrists, prompting production to temporarily shut down.
- Bruce Willis personally paid the hotel bills for his injured stunt double's parents and visited him multiple times during his hospital recovery.
