• Director Darren Lynn Bousman was brought on after pitching a rejected script called The Desperate, which Lionsgate executives found stylistically similar enough to serve as the foundation for Saw II.
• Shawnee Smith's contract for Saw II included a $150,000 base payment plus a $100,000 bonus if the film grossed over $50 million.
• Only key cast and crew members working on the film's ending received the complete script; others got only the first 88 pages, and rewritten pages were shredded to maintain secrecy.
• Four or five alternate endings were shot to keep the actual ending a surprise.
• The Needle Pit trap required the production team to modify 120,000 syringes by replacing their metal tips with fiber optic points over a four-day period.
• Erik Knudsen filmed his scenes while ill with the flu, which he noted actually suited his character's condition of being poisoned.
• Production designer David Hackl built 27 sets on a single sound stage in just three weeks.
• Beverley Mitchell, who disliked horror films, accepted the role of Laura Hunter as a personal challenge despite struggling to watch even the first Saw film.
• Tobin Bell was not contractually obligated to return as Jigsaw but chose to reprise the role.
