- Dennis Lehane drew inspiration for Shutter Island's hospital and island setting from Long Island in Boston Harbor, which he visited as a child during the 1978 blizzard.
- Columbia Pictures initially optioned the rights to Shutter Island in 2003 but never developed the project, allowing the option to lapse back to author Dennis Lehane.
- Martin Scorsese and Leonardo DiCaprio both independently became interested in the Shutter Island project.
- Production on Shutter Island began on March 6, 2008, and wrapped on July 2, 2008.
- Old industrial buildings in Taunton's Whittenton Mills Complex were used to replicate the Dachau concentration camp for the World War II flashback scenes.
- To film director Cawley's office scenes at the abandoned Medfield State Hospital, the crew shot on the chapel's second floor during late evening and shone lights through windows to simulate daytime.
