- Sandra Oh was initially asked to audition for the role of Dr. Bailey but insisted on reading for Dr. Cristina Yang instead, the part she ultimately won.
- Rob Lowe turned down the role of Dr. Derek Shepherd because he had already committed to starring in the medical drama Dr. Vegas.
- Justin Chambers was added to Grey's Anatomy as Dr. Alex Karev through digital editing and additional scenes after not appearing in the original pilot.
- Shonda Rhimes employed color-blind casting for Grey's Anatomy, with roles cast without predetermined racial descriptions, resulting in a racially diverse ensemble.
- Eric Dane auditioned unsuccessfully for the pilot episode but later joined in Season 2 as Dr. Mark Sloan, initially as a recurring character before receiving star billing in Season 3.
