- True Beauty was adapted from a Line Webtoon series, with the television adaptation announced in July 2019.
- Moon Ga-young, Cha Eun-woo, and Hwang In-youp were cast in the lead roles in August 2020.
- The original webtoon creator Yaongyi designed the male lead character Lee Su-ho with Cha Eun-woo specifically in mind.
- Filming was temporarily halted in late December 2020 after cast member Kim Byung-chun tested positive for COVID-19.
- The first promotional stills were released on November 10, 2020, about a month before the series premiered.
